Lost Canadian Identification

If you have a child that was born in Canada, or family members that are Canadian citizens or Permanent Residents, and they have lost their Canadian identification, you can contact the following agencies. An ID worker will fill in the forms with you, order the ID for you and provide a secure address at which you can receive it.  There is no charge for the ID or for this service.

Service description:
Nursing Care - Staff nurses, working with volunteers, operate scheduled clinics in drop-in centres and shelters. Nursing outreach is designed to locate people living outdoors, on the street - people who tend not to go to the locations where scheduled clinics occur.
Mental Health Support - For clients who also have mental health problems we provide intensive support, including crisis intervention and case management.
HIV/AIDS Prevention - Our clients are given preventive education, assistance with health problems and referral to other services. Condom distribution, a needle exchange and street outreach services are part of this program.
I.D. Replacement - We help our clients to obtain birth certificates, Social Insurance cards, Ontario Health Cards, landed immigration documents, and other documents necessary in dealing with modern bureaucracies.
Secure Mail Service - Homeless people by definition lack the postal address required to qualify for certain services. Street Health will provide the necessary mailing address and handle a person's mail.
I.D. Storage - The constant problem of loss or theft of identification documents among homeless and underhoused people is addressed by providing a secure, managed location for the storage of such documents.
Hepatitis C Support - This program provides support for homeless people with hepatitis C (Hep C). Services include Hep C education, referrals and accompaniment to health and social service appointments, the provision of supportive counselling, harm reduction supplies and information, and a weekly support group. The program also offers workshops and information on Hep C to a wide range of service providers in Greater Toronto.
Clothing and Sleeping Bag Distribution - Clean donated clothing is distributed, as are new and professionally-cleaned used sleeping bags.
